TMP- Phase II (2006 -2009)
Fourth Quarter Partner Report Form
(April 1 - June 30, 2009)

Thank you for taking the time to complete your quarterly report. We appreciate the work you are doing!

IN THIS QUARTER'S REPORT IT IS ESPECIALLY IMPORTANT THAT YOU GIVE US AS MUCH UP-TO-DATE DATA AND BUDGET INFORMATION AS POSSIBLE. WE MUST BEGIN THE CRITICAL PROCESS OF WRITING FORMAL END-OF-PROJECT REPORTS TO GATES AND THE LEGISLATURE.

We ask you to be as "newsie" as you can, using specific examples and anecdotes from teachers and students.

NOTE: You are required to submit a Fourth Quarter Budget update separately from this report. Upon completing this report form, you will be sent back to the Deadlines and Due Dates page to access the Budget Form.
